The intake forms below are for use in Family Law, Probate, Estate Planning, and Family Mediation cases. Please fill out the appropriate form and return it to my office prior to your consultation, so that I can review the form and formulate my initial advice. Your information will be kept strictly confidential. If you have questions along the way, don't hesitate to contact me. Don't be intimidated by the length of these forms. They are very broad in scope and many questions will not be applicable to your situation. Just fill out what you are comfortable with and I will let you know if additional information is necessary.
